Problem with IO-540 mag drive coupler bearing.

Let's start buy going back 2 years ago. It was my first annual with my new to me PA-32-300, I had put on 25 hours since I picked it up. when we checked the screen we found parts of the left mag drive coupler bearing in it. My IA pulled the base of the engine and we found all balls and pieces of the cage. So we replaced the bearing and reassembled the engine and completed the annual. I ran the plane for another 25 hours and changed the oil and checked the screen and found no pieces. Well since then I put a little over 100 hours since then. Now at the second annual since then we found the same bearing had failed again. My IA has talked to Penn Yan Aero and lycoming directly and both have said this is not a common issue, and think it might be a fluke. I don't want to take the chance of it not being a fluke since this is a plane I fly my family around in a lot. So I'm just wondering if any else has seen any issues like this or might be able to point me in a different direction.
